Form 1583 Instructions - Print These for Future Use

Print the 1583 document. Select "Shrink to Fit" in the print dialog box. If you have problems printing, call us at 310-823-7802 and we will fax you a copy of the 1583 form.

Info Box#	Information Required
#1	Put today's date
#2	Put the name of the person or business who/which is going to receive mails using this mail box.
#5	Put the names of the person you wish to give authorization to pick up your mail from the store.
#6	Put your name as an account holder
#7	Put your address and phone number.The address should match the address on your valid photo ID.
#8	Put the types of IDs you are going to send us the copies of.Ex: California Driver's License, Insurance Card, Utility Bill, Student or Corporate ID.
#9	Put the name of business which is associated with this box.
#10	Put the address and phone number of the business.
#11	Put the type of the business.
#12	Put the additional names of people who will be receiving the mail using this box. You can put 1 name for a small, 2 names for a medium, and up to 4 names for a big box.
#13,#14	Put the valid explanation for each box.
#16	Put your signature
You need to include copies of the required IDs for each person stated in Boxes 2, and 12, and for yourself for the box 8.

Instructions To Notary Public

1. The customer must supply you with 2 forms of identification. At least one of these needs to be government issued photo ID. Please list these in section 9, lines a & b. Be sure to write the ID. numbers down, as well as the ID that was used. ( Example: California Driver's License #0000000)

Acceptable forms of Identification: (As defined by the Postal Service):

Valid driver's license

Valid state issued ID

Armed Forces ID

Government issued ID

University ID

Recognized Corporate ID

Passport

Alien Registration card

Current lease, mortgage, or deed

Voter registration card

Utility bill

Home or vehicle insurance policy

Vehicle registration card

*Social Security numbers, credit cards, and birth certificates are not acceptable forms of identification.

2. Signer's address in section 8, must match the identification. If it does not, signer has been instructed to supply you with proof that they reside at the address. (This can be a utility bill, a lease, voter registration card, etc.). If you use this, please indicate on the bottom of the form what was used.

3. Please sign Section #16
